Registration Form

To be officially registered for a class, you will need to click on the link to the fill-in form below and complete the Registration Form so I can hold a place for you in the next class.

This is a fill-in form

On your computer, open it (Not download it) and fill it out, then do a "save as" with your name added to the filename (CCW-Class-Registration-Form_Your Name.pdf)

then email it back to me so I know you are coming to the class.

I need to have a filled in form from each person coming to class to teach you, mail you your photo after class and be able to contact you quickly if anything changes.
